 | | If you plan to play Division I or II athletics, an NCAA Clearinghouse form must be completed. This form is ONLY available online at the link above. Please complete the form, which requires a payment of $50 with a valid VISA or MasterCard credit card. Once the form and payment information are completed, please continue to the next screen where you will need to print up to 2 copies of the Transcript Release form also on this Web site. Please return the 2 NCAA Clearinghouse Transcript Release forms along with a pink transcript request form to the college counseling office. Upon receipt of the NCAA Clearinghouse Transcript Release form and pink transcript request form, the student's transcript will be sent to NCAA.
